Актуальні питання медико-соціальної реабілітації учасників бойових дій : досвід та перспективи

Бойові дії на сході України є причиною реорганізації медичних та реабілітаційних закладів. Наслідки перебування в зоні військових конфліктів проявляються психосоматичними захворюваннями. Мінно-підривні травми призводять до морфофункціональної патології, посттравматичних стресових розладів та порушення адаптивних механізмів організму. Розглянуто фізіологічні причини дезадаптації та нервово-психічних порушень з метою зосередження уваги на необхідності комплексної реабілітації постраждалих. Наразі багаторівневий комплекс саногенних заходів полягає в застосуванні медико-психологічних, психотерапевтичних, медикаментозних і немедикаментозних методів. Інтенсивніше впроваджують іпотерапію, каністерапію, озонотерапію, що має здатність шляхом неспецифічного впливу стимулювати захисні адаптивні реакції організму. В зв’язку з формуванням поєднаної патології в організмі учасників бойових дій залишається потреба в удосконаленні комплексної допомоги по лікуванню та реабілітації.

The fighting in the East of Ukraine caused the reorganization of institutions for the provision of medical care and rehabilitation of the population who need them. The article discusses the best practices of rehabilitation specialists in some countries of the world. It is revealed that every fifth participant in hostilities in the absence of any physical damages suffers from neuropsychiatric disorders, and in their presence – each third. The consequences
of being in the zone of military conflicts are manifested in psychosomatic illnesses. Compared with healthy people participants in hostilities have in 2-3 times higher probability of occurrence of pathologies of the nervous, cardiovascular systems, digestive organs. Mine-blasting traumas, which lead to morphological functional pathology, cause the formation of post-traumatic stress disorders and violation of adaptive mechanisms of the body. The article discusses the physiological causes maladaptation and neuropsychiatric disorders for the purpose of focus on the need for complex rehabilitation of the victims. It is discovered that 54% of all appeals of demobilized soldiers need medical rehabilitation, physical – up to 30%, psychological – more than 80%. Medicinal therapy is not always effective because of side effects and addiction to drugs. More intensely introduce hippotherapy, canisterapy, ozonotherapy, which have the ability due to nonspecific effects stimulate the protective adaptive reactions of the organism. The multilevel complex of sanogeneous events consist in the application of medical-psychological, psychotherapeutic, medical and non-drug methods. Highly specialized medical care is provided by the clinics of the National Academy of Medical Sciences of Ukraine, hospitals, rehabilitation centers, etc. In connection with the formation of combined pathology in the body of participants in hostilities remains the need to improve comprehensive care for treatment and rehabilitation. Now is relevant the training of qualified rehabilitologists specialists and creation of highly effective complex programs of medical, physical, psychological and social rehabilitation of victims. Taking into account that there are no methodological principles in the organization of complex rehabilitation of the victims of hostilities, and rehabilitation takes place in separate theoretical and practical directions, it remains necessary to develop the optimal ways of carrying out complex medical and preventive rehabilitation measures.
